1. Problem-Solving Procedure: We believe that most problems can be resolved informally with a conversation. However, we do recognize that there may be occasional misunderstandings and problems that will need more formal attention. We Want to resolves these types of situations in a fair and timely manner. In order to do this, we need your help in bringing them to our attention. No problem is too small or insignificant, and each issue will be given the utmost attention and consideration.

2. Employee Request Leave: When taking a leave of absence, for any reason, including PTO or medical leave, and Employee Leave Request Form, must be submitted to and approved by your immediate supervisor. Employee Leave Request Forms should be submitted at least thirty (30) days prior to the scheduled leave date, unless the request is due to an unexpected emergency.

When returning to work from a medical leave, all employees must present a doctors release.
